First Visit

On your first consultation with Dr Low please arrive 15 mins prior to your appointment so you can fill out paperwork or alternatively click here to download, print and fill in prior to your appointment with Dr Low. You will need to bring:

• A referral from your GP or other medical specialist (this is so you can claim part of the consultation fee from Medicare)
• Your Medicare card
• Private Health Insurance card
• DVA card if applicable
• Your x-ray’s, MRI’s or CT scans relating to your injury
• Cost of consultation which is payable the day of consultation
